Creekside Day Treatment is a collaborative program of Hillside Children’s Center and Monroe No.1 BOCES. It is a unique regional program that offers academic and vocational education in combination with mental health treatment for children with dual diagnoses. Students are referred to Creekside Day Treatment by the Committee on Special Education in their local school district. We accept youth ages 5 to 21 years of age who have an intellectual disability and severe mental health challenges. We serve students in Monroe and surrounding counties.
About this Program
We work with families and local school districts to develop a customized individual plan for each student. We assign youth to one of our classrooms based on age and clinical needs. We provide:
- Academic and social skills development
- Community-based instruction and life skills curriculum
- Comprehensive psychiatric care
- Individual, group, and family psychotherapy
- Vocational training and workplace readiness
- Psychological evaluation and consultation
- Crisis intervention
- Case management
- Behavior management
Our multidisciplinary team includes special education teachers, master’s prepared clinicians, psychiatrist, psychiatric nurse practitioner, psychologist, crisis intervention specialists, case manager, behavioral consultants and specialized therapists as needed.
Because our students’ complex needs extend beyond school age into adulthood, we help them transition to the most appropriate adult programming to meet their ongoing needs.
Parents are partners in every aspect of our program, from initial planning to regular follow up meetings and at-home visits. We succeed by working hand-in-hand with families, and we are committed to keeping them involved in every aspect of their children’s care.
Eligibility
Youth ages 5 to 21 years
Open to eligible residents of the following counties: Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Wayne and Wyoming.
